Horizons Home Care is a division of Hospice of Wake County. It is a private, not-for-profit home health agency that delivers the highest
quality
skilled care to individuals needing medical assistance at home. Under the direct supervision of a physician, our teams develop a care plan to help you remain as independent as possible.
Unlike hospice care, you could be seeking life-prolonging or curative therapy. Our staff is dedicated to promoting the maximum level of health and independence possible for every individual we assist. At Horizons Home Care, our goal is to enable our patients to function independently as quickly as possible.
Horizons Home Care is:
- Medicare certified
- Member of the Association for Home and Hospice Care of North Carolina
Horizons provides in-home care to individuals:
- Who have a skilled nursing need
- Who are homebound
- Who have a personal attending physician
Horizons Home Care:
- Assigns a nurse case manager who oversees and coordinates all care, under the patient's physician orders.
- Arranges for the delivery of necessary medical equipment - wheel chairs, hospital beds, etc.
- Coordinates administration of prescribed IV medications with the infusion company.
- Arranges for speech therapy, occupational therapy or physical therapy as ordered by physician.
- Coordinates additional services with other community agencies that may be able to assist the patient.
Horizons Home Care provides:
- Admission to services within 48 hours of referral
- 24-Hr. On-call Medical Assistance
- 15 minutes or less response time to patient or physician phone calls
- Home Health Aides for personal patient care as needed
- Medical Social Workers as ordered by the physician to meet special patient needs
Horizons Home Care offers to both patients and physicians:
- Negotiation of competitive pricing for needed medical equipment, trained therapists, etc.
- Negotiation for coverage of services with insurers and HMOs
- Filing claims with Medicare, Medicaid or private insurance
- Assisting physician's business offices to facilitate reimbursement
