Team Leader

This is a full-time professional position responsible for coordinating patient care staff to ensure day-to-day admission and continuity of care for all patients on service and ensure accountability of patient care consistent with the agency’s mission statement and regulatory systems.  This position works closely with other Team Leaders and staff.  RN/MSW/Chaplain with current N.C. license, as applicable with previous supervisory experience, 5 years recent clinical experience in hospice and/or home health or equivalent and proven ability to educate staff

Facility RN Case Manager

Full-time professional position to assess, admit and provide case management for patients assigned in a facility setting. This position works to provide optimum patient comfort as part of an interdisciplinary team under the attending physician’s order consistent with the agency’s mission statement and regulatory systems. Candidate must be a Registered Nurse with current NC license; a BSN is preferred. The ideal candidate will have a minimum of 2 years recent clinical experience. Hospice and/or home health or nursing home experience or equivalent is preferred along with Hospice Nurse Certification.

Clinical Preceptor

This is a full time professional position that provides clinical oversight, orientation, and support to newly hired clinicians during the introductory period of employment while carrying a teaching caseload. This position is available to assist with clinical skills development and questions from established Case Managers. This position requires a Registered Nurse with current N.C. license; BSN and CHPN preferred. A Minimum of 2-3 years clinical experience is required in a health-related field; Hospice and/or home health experience or equivalent is preferred. Working knowledge of NC Licensure Law, Medicare Conditions of Participation, and all accreditation standards is needed. Excellent communication skills, verbal and written along with computer skills including experience with Microsoft Office applications and excellent administrative, organizational and leadership skills are preferred.

Weekend Float On-Call (Baylor)
Full or part-time professional position responsible for making scheduled weekend visits, assisting On Call Nurses, unscheduled and admission visits, answering patient phone calls; may work Friday, Saturday Sunday or Saturday, Sunday Monday; Registered Nurse with current NC license; BSN and/or Certified Hospice and Palliative Nurse Credentials preferred; Minimum 2 years recent clinical experience; Hospice and/or home health experience or equivalent preferred; Excellent verbal and communication skills; Computer literate; Work effectively and calmly during a crisis; able to make decisions; Must have access to transportation.

Home Health Aide

Part-time Home Health Aides needed for evenings and weekends.  Home Health Aide will provide personal care for patients including but not limited to giving evening meals and evening tuck-in as consistent with the Home Health Aide Care Plan and with the agency mission statement and regulatory systems. Current listing card with NC Home Health Aide Registry required; Hospice or home health experience preferred; must have access to transportation.

Informatics Coordinator

Full-time professional position to provide clinical information system support. The Informatics Coordinator will act as a liaison between clinicians, the facility, and the IT team to communicate user needs and facilitate system modification to meet user needs. This position will also be responsible for development, coordination, and implementation of user training and user support for all of clinical computer systems within the agency. This position will collaborate with the Clinical Department and the IT Department. Applicants should be Registered Nurses with current N.C. license; a BSN is required; a Masters in Nursing Informatics is preferred. The ideal candidate will have a minimum of 5 years recent clinical experience. Hospice and/or home health experience or equivalent is preferred. A Minimum of 2 years Informatics or IT experience is preferred along with experience teaching clinical computer documentation.

Reflections Team Leader

Full-time professional position that develops, plans, organizes and implements Reflections (Children’s Bereavement) Program, collaborating with the Bereavement Program Manager. This position is a resource to the agency and the community on children’s grief and bereavement issues.  Requires travel and professional networking as part of community outreach and may require irregular hours, including some evenings. Master’s degree in an appropriate health/human services field with LPC or LCSW certification; Expertise in grief and bereavement, particularly as relating to children; Hospice experience preferred; 5 years clinical work experience; Excellent written and verbal skills.

Communications Specialist Part-time

Part-time position to design and produce printed materials to support public relations, marketing, and fund-raising activities of the agency in a 20-24 hour work week: Five 4 hour days or four 4 hour days and one 8 hour day; Bachelor’s degree from an accredited college or university in graphic design, art, advertising, or related field; or currently working toward the latter; Minimum of 2 years work experience preferably in graphic design; Proficient in Adobe InDesign, Illustrator & Photoshop. Microsoft Word, Excel, PowerPoint, Access and Internet.
