For hospice services, palliative care consultations or information visits, please call our Access Department at 919-828-0890, Monday – Friday, 8am to 6pm. This number can also be called for evening and weekend referrals.

Additional information needed for a referral:

Please fax the following information to our Access Department at 919-828-9514:

1. Doctor’s order for a hospice assessment
2. Signed certification from attending physician stating that the patient has a life expectancy of six months or less
3. Recent history and physical
4. Medication list
5. Other pertinent information that will help us care for your patient
