To make a referral for Horizons Compassionate Care,  Horizons Home Care, Horizons Palliative Care, or Hospice, please call 919-828-0890 and speak to a member of our Access Department. After normal business hours or on weekends, you may still make a referral by calling 919-828-0890 and speaking to a member of our on-call team.
